Information we collect

When you submit our form, we collect your name, the email address or phone number you give us, your message, and the page you sent it from. We also collect standard technical information (browser type, pages visited) through analytics tools.

How we use it

We use your contact information only to reply to your request with general information, to act on a correction, or to handle a deletion request. We do not share your submission with any treatment provider, we are not paid for referrals, and we do not sell personal information. The owner of this site also has an ownership interest in a behavioral health treatment provider; that provider does not receive your submission. Ketamine Treatment Guide is an educational resource, not a treatment provider — we do not provide medical care, medical advice, diagnosis, insurance verification, or placement.

Sensitive information

We understand that contacting us can involve sensitive health context. Please share only what you're comfortable sharing in the message field — a request can be as simple as your name, an email address, and your question. This website is not a healthcare provider, and information you submit through it is not protected by HIPAA.

Service providers

We use a small number of services to run the site. Web3Forms delivers form submissions to our email inbox. Ahrefs Analytics measures site traffic. Cloudflare hosts the site. These services process data on our behalf.

Data retention and your choices

Form submissions are delivered to our email inbox through Web3Forms. We delete them from our inbox and from Web3Forms within 90 days after we reply, or sooner if you ask. You may ask us to delete your submitted information at any time through our contact page, and you can tell us to stop contacting you at any time.

Your privacy rights

Depending on where you live, you may have the right to confirm whether we process your personal data, to access it, to correct it, to delete it, and to obtain a copy of it. We do not sell personal data, use it for targeted advertising, or use it for profiling. To exercise any of these rights, use our contact page. We respond within 45 days and will tell you if we need one 45-day extension. If we decline a request, you may appeal by replying to our decision or by sending a new request through the same page with "appeal" in the message; we answer appeals in writing within 60 days. If we deny an appeal, Connecticut residents may submit a complaint to the Connecticut Attorney General at portal.ct.gov/ag.

We do not collect, use, or sell personal data for the purpose of training large language models.

Cookies and analytics

Our analytics service helps us understand how visitors use the site so we can improve it. You can control cookies through your browser settings.
